What is Social Prescribing? Social Prescribing aims to connect people with activities, groups, and community resources that meet their interests and needs to improve their health and wellbeing. It combines medical care with social support, helping people feel healthier and more connected. 

To learn more, drop by our Explore Social Prescribing event with fun activities, coffee and snacks, and chat with our Community Navigators and see how you can get started.
